crazy bolted connection

I make a very simple bolted connesisone aligned to a hole, I see it well previewed during creation or modification, but then it places me from anywhere and out of axis (you see it in the annex, it is positioned where it is semi-transparent). I tried to do it and do it again.
Any ideas?

usually when there are recurring malfunctions and no explanation, the first thing I do is restart the pc.
