I have to make an offset of a fairly complex surface (in the annex you find the png to realize); when I do the 1mm offset control to the inside creates a surface full of "holes". What advice to improve the offset?

as falonef says, first check that the entire surface is closed, then you can try using automatic smoothing and if you master the values you can set the manual smoothing (maximum deviation lower than the offset value), but the command cannot do miracles if geometry is incongruent and forcing it risk to get a surface that presents "straps" that is, it seems good but if you analyze it is not.
such a complex surface requires that you reconstruct the surfaces that the command proposes to remove, besides I would act for zones (before the flat parts, then the bugs, then the alettatures etc.) controlling the result and creating the new surfaces of union between the parts.
I did so: multiple extraction of one surface, offset of that surface, I was part of the extraction command and select the adjacent surface, this allowed me to see the result of the offset as I added the surfaces and visualized without smooth edges I identified the problematic zones
